Our goal is to reduce the complexity and gaps in the healthcare system with real-time and on-demand virtual services and support for our most vulnerable populations. By directing people to the right services at the right time and diverting them away from crisis and unnecessary treatment (i.e. ED visits for routine medication refills), we are providing patients with better access to higher quality care when they need it the most.
Our virtual team offers on-demand access to multilingual virtual care via phone, video, text, and chat:
- Coordinated care — care coordinators, medical care workers & supporting care staff
